What is transanal endoscopic surgery?

Transanal endoscopic surgery is a minimally invasive procedure that enables the surgeon to treat conditions without the need to make any incision on the body.  The anus, a natural orifice, is used as an entry site to gain access to the large bowel (the colon and rectum) [Figure 1].

Specialized rigid or flexible instruments are used to perform the procedure inside the large bowel [Figure 2].  The surgeon uses specialized equipment including a high definition television screen [Figure 3].  It is scar free surgery with the advantages of faster recovery and less complications.  Most patients are discharged the same or following day and can resume work after a short recovery period.

What conditions are treated with transanal endoscopic surgery?

Some of the indications for transanal endoscopic surgery include relieving blockage, treating bleeding, and removing tumors.  Conditions that can be treated include the following:

  • Colorectal fistula
  • Early colorectal cancer
  • Colorectal polyps
  • Narrowing of the colon and rectum
  • Tumors of the colon and rectum

Transanal endoscopic surgery consists of numerous procedures including Transanal Endoscopic Microsurgery (TEM) or related techniques such as TAMIS and TEO, and therapeutic endoscopy procedures such Endoscopic Mucosal Resection (EMR), Endoscopic Submucosal Resection (ESD or ESR), endoscopic stent placement, and endoscopic dilation/treatment of strictures (narrowing).
